Nurse Practitioner Model
The Sharecare Nurse Practitioner model is patient-centered, with a focus on assessment, diagnosis and glycemic treatment of the patient hospitalized with diabetes. Nurse Practitioners use evidence-based approaches to develop institutional guidelines and/or care pathways to optimize and manage glycemic goals while preventing adverse events. The Nurse Practitioner can effectively support a decrease in the patient length of stay and diabetes related complications that increase hospital costs. In addition, this model supports strategies to transition the patient to the post-acute environment or post-acute or home care, thereby minimizing the risk for rehospitalizations.

Diabetes Self-Management Education and Support
Diabetes Self-Management Education and Support (DSMES) is an instructor led learning curriculum that teaches people living with diabetes how to effectively take control of and manage their condition. The comprehensive curriculum developed and updated by Sharecare leverages national standards to educate people with diabetes on key concepts and lifestyle best practices to manage diabetes.

Medical Nutrition Therapy
Medical Nutrition Therapy (MNT) is delivered by a Registered Dietitian Nutritionist. This program is a comprehensive service that involves individualized nutritional assessments and group education. The program helps participants establish goals, nutrition plans, and supports behavior change. MNT and DSMES are complementary services and more effective when offered together.
DiaWEB is an integrated, browser-based platform developed to support you in managing the infinite details, so you can focus on what’s important: the patient. Utilized throughout healthcare systems in the U.S. and Canada, DiaWEB puts comprehensive referral, participant, scheduling and program management tools at the fingertips of your entire multidisciplinary diabetes support team. It also provides real-time access to aggregate data for program evaluation, benchmarking and quality improvement. Plus, you have the additional option of using the HL7 interface to connect directly to your electronic medical record (EMR).
